Your Blood Pressure Device
Your healthcare provider has recommended that you monitor your blood pressure at home. Checking your blood pressure is simple using your maternity support app. It is offered through your OB clinic and powered by Wildflower. From the app, your Wildflower Health Advocate can share your blood pressure readings with your OB clinic.

Your Blood Sugar (Glucose) Device
Your healthcare provider has recommended that you monitor your blood glucose (sugar) at home. Blood glucose monitoring is simple using your maternity support app. It is offered through your OB clinic and powered by Wildflower. From the app, your Wildflower Health Advocate can share your blood glucose readings with your OB clinic.
Important things to know about test strips:

Only use test strips that are meant for your specific glucose meter.

Wildflower Health Advocates and your Maternity Provider
Wildflower Health works closely with your OB to support your pregnancy and postpartum journey. That includes recommended or prescribed remote patient monitoring. (This is also known as RPM.) It also includes devices like a blood pressure device or blood glucose device. This will come enabled with Bluetooth. That way, it can sync with your app.
You will have your own Wildflower Health Advocate. They’ll be here to answer your questions about your health. They can also connect you to resources to help you have a healthy pregnancy and baby. Your Wildflower Health Advocate also works with your OB provider. They’re a trusted member of your care team.

Why am I getting this device?
You have been prescribed either a glucometer or a blood pressure device by your maternity care provider. This device will help monitor a condition during pregnancy. It may also be used after you deliver your baby.
How long will it take for me to receive my device?
First, our Health Advocate will reach out to you to review your care plan. After that, you will get your device within 5 business days.
What is remote patient monitoring?
It's a way for your maternity care provider to keep track of your health in between your visits. It lets them see your blood glucose (BG) and blood pressure (BP) measurements. You may get a call from our team If you have an out-of-range measurement. (This could mean either high or low number.) We may offer:

Is this going to cost me money?
This program is completely free to you. It's been ordered by your maternity care provider. So, it's covered as part of your care.
